Complete Vestibular Stenosis: A Real Challenge in Rhinoplasty

Background

Complete vestibular stenosis is very unusual in clinical practice, and managing such a case with rhinoplasty is challenging because of the recurrence of disease. In the present case, we have enlightened the management of a rare and complex case of complete unilateral vestibular stenosis.

Case Report

A 23-year-old young man presented with complete vestibular stenosis (left side), which resulted after a septal surgery. The vestibule was reconstructed with radial skin flaps, and the cartilaginous framework was strengthened by costal cartilage graft. Patients had adequate nasal patency and good aesthetic outcomes till 12 months after the surgery.

Conclusion

Iatrogenic vestibular stenosis is very unusual in clinical practice, and managing a case is quite challenging because of the recurrence of disease. Preserving the vestibular skin and good structural support to the septum and the lateral alae are key surgical steps in preventing restenosis and improving functional and aesthetic outcomes.
